So based on that shouldnt the answer be B and D? The answer mentions clearly that this is where transcription begins...
 
I would have thought the same.

When they say "Start site," I guess they mean the site where the first nucleotide of RNA is created.
 
could be a typo, but if this question came on the DAT I would pick D because it is a better answer than B

Destroyer is correct here. You are confusing transcription initiation site with a promoter site. The question is asking for a promoter site. This is a site where RNA polymerase binds before RNA begins to be synthesized ( made). Transcription start site, is the series of nucleotides on the DNA, where RNA begins to be assembled. Hope this helps.
