Did you grow up in a rural area? What activities were you involved in during high school (granted, you won't be listing them in AMCAS, but curious)? If rural health/community is important to you, I'm not sure going to an Ivy+ is helpful unless they are specifically associated with an AHEC or rural health community organization.

Would you apply to the NHSC? HPSP?


Your in-state is always going to be your best bet, so check out Indiana.

As for schools that have strong research infrastructure but likely opportunities in rural health, I suggest looking into the Michigan schools like Michigan State and Western Michigan. Iowa and Minnesota are also strong with research, and you could check out Creighton. Maybe your Kentucky schools like Louisville and UK are worth looking into. If you want a different "rural," maybe check out Arizona, Colorado, and Utah schools.
 
You have a good list and should receive several interviews. UAB admits few non residents with no connection to the state.
