Diagnoses in Patient Problem List:ROUTINE MEDICAL EXAM[V70.0],ROUTINE GYN[V72.3]

In viewing patient EMR's in Epic and Logician/Centricity, I've noticed that patients in their "problem list" sometimes have listed the 'problem' of ROUTINE MEDICAL EXAM [V70.0], or ROUTINE GYN EXAMINATION [V72.31].

Can anyone explain this phenomenon to me (why those are diagnoses, why only a few patients who see their primary care physician have these when I'm sure many people are getting routine medical exams, any other information on the topic).

V70.0 is a billing code and the bill is generated for a "routine medical exam". If someone has no other problems. "routine medical exam" may be the best choice for the medical coder.

Depending on where you are, many of the people seeing the doctor are coming in for specific complaints or for follow-up of a specfic chronic condition. While periodic follow-up of a chronic condition may be "routine", the probem is going to be hypertension, or congestive heart failure, or diabetes, etc.
