you have to do a residency in fantasy followed by a fellowship in jackassery.
I'd say it's Emergency Medicine. I'm surprised none of the EMs have spoken up. Basically, by the time the patient is sent up to the floor, there is an admitting diagnosis.
Nothing against EM, but they're just about the furthest thing from a "House, M.D."-style diagnostician there is, unless you count "sick" and "not sick" as diagnoses. 😉
Personally, I'd vote for renal...those guys know freakin' everything.
Well, we don't cover the fibromyalgia tenderness points like FP does, but . . .
Let's not let this devolve into a specialty bashing thread.
I think my diagnostic skills are just fine and I rarely send someone without a decent clue of what's going on.
I can tell as many stories about the FPs or internists who sent "sick" patients to the ED, the transfer papers still wet from their sweat and tears.
I've had more than one patient call me "House." It's my pleasing disposition.

I've used this example before. Medicine is like a golf course. EM docs tee off and drive. Our job is to get it on the green. Sometimes, we're on the fairway. Sometimes, we're in the rough. Rarely, we're off the course. Occasionally, we'll hole out.
However, generally, specialist-putters come in. Sometimes, the IM/FM/Surgery putters are snipers, and need one shot. Sometimes, the specialist-putters will, well, putter around, and 3-, 4-, 5-putt or more.
Medicine is not a "zero-sum" game - I'm not good because you suck.
And most cases aren't "Pebble Beach" quality - most cases are the most-straightforward, boring, not-challenging holes that have 1 sand trap, no dog leg, no water hazard, and are par-3. I make "actual diagnoses" all the time.
