ROCR the least of our concerns at this point. Will be legislation by FIAT and social media pressure.
Trust me...no one who actually works wants an absolute emphasis on efficiency.
Not me. I like a world with multi-disciplinary meetings, CME time, research offices, time with patients, academics (not me, but good to have around somewhere), non-profit directed research (fundamental stuff overwhelmingly funded by gvt), opportunities for scholarship for those that want to pursue it and an emphasis on employment for the public. These are called human values.I dunno, those of us in private practices who are able to deliver care at a fraction of the price of hospitals and academic centers think we'd be able to play rather well in that sandbox.

I cannot imagine it being very popular within any region (unlike aid to farmers, disaster relief and aid for bridge reconstruction) nor high priority politically for any congressman. There is no ideological motive for it (on the right or left).
