Where'd you get the $45k difference?
Total COA at Houston, San Antonio, Baylor is roughly: 224, 222, 202.
Total tuition and fees at Houston, San Antonio, Baylor are roughly: 109, 100, 102.
Haha I just went looking for the financial aid packet from UTSD... nothing, just Baylor and SA. Luckily the UTSD website has the breakdown.
I personally won't be reapplying til this coming spring but it would be awesome to meet you great folks during interviews and whatnot.

Haha yea after my interviews I filed them according to finances, curriculum, student orgs, etc. so I have a better idea of the differences between each school. I just remembered looking at ~200k vs ~245k, but like I said, I might have written down the wrong number. Figured tuition for Houston is around 125-130k and add another 100k for living is closer to your estimate. Of course this won't even really matter if I don't get to pick.

From the information the school gave me from the interviews
Baylor- 202,173
UTHSCSA-223,000
Houston- 242,000
With living expenses.
I think its kind of odd because for baylor they have the living expenses as...
year 1-24,786
year 2-27,951
year 3-27,111
year 4-29,092
I don't know why there is such a big diffrence in living costs between D1 and D4.

Well now the wife and I are thoroughly confused... two people now that have quoted 242k, but based on their COA handout it adds up to 224, including the $25k/yr living expenses (20k in D4).

Regardless, the actual tuition and fees of the 3 schools are very close to the same, from there COA will vary widely by person.
EDIT: Nevermind. As catoshiro pointed out below, this is EY2015, EY 2016 has a $10k tuition increase not shown here.

Well here's the exact number for estimated tuition and fees for entry year 2016-2017. I remember Dr. P saying they increased tuition. It is now $138,802 for incoming students. I think that table says tuition and fees is around $128k. Makes sense because it's for EY 2015.
So $93,233 for Baylor, and $114,552 for SA to compare just tuition and fees. Living expenses is variable so COA can be higher or lower between students at different schools.

A 10k increase in one year?!😱 That's crazy.
I guess; I thought I heard him say 5% increase but it's a little more than that. So yea, idk. Only estimates though, maybe a rich benefactor like the UT Houston Med school guy will come out and save people who get accepted and go to Houston lol.
I remember him saying something about there being a freeze on tuition increases after the 10K jump this year, so that we wouldn't have to worry about it increasing. Not sure how air tight that is though...
I know right? Must be some sort of clause somewhere if you accept the offer. Tuition rates freeze for EY 2016 for your four years but could increase again the next year for EY 2017. So you could be paying less for your D1 than the class after you. At least that was my understanding.
This is a very ignorant question but why does tuition rise? Lack of funds? Increased costs of supplies?

I thought that donation was for UThealth in general.
I think just the med school. They even renamed the med school in honor of the benefactor. Now it's the McGovern medical school.
