I have been able to formulated DD for 15 or 20 of the most common presentations that present in the Internal/Family Medicine office and the ED. I have been trying to do that with back, limb pain, neuropathy, myopathy and so forth but have been trouble finding a text that seems to help? I was wondering is their anything out there that helps me to frame it that way?

Don't know of any text, but go back to the old med school mnemonic and play the game yourself. The ddx is to spark ideas which sparks inquiry whether it be testing, examination, history or hitting the library. Also being able to verbalize a broad differential for a chief complaint will get you points on your PM&R oral boards.
