Disclosure of medical history

I recently got off the waitlist at a school, and with the materials they sent for matriculation there is a "History and Physical Illness" form that has to be signed off by my primary care physician. The form includes disclosing any current acute/chronic illnesses, history of acute/chronic illnesses, and mental health conditions. It is separate from immunizations, physical exam, and provider attestation of ability to perform the required actions, as these are separate forms.

My question is, are medical schools allowed to require this? I do technically have a disability, but was not planning on disclosing this as it does not affect my academic performance or ability to succeed in medical school.
