I'm currently a volunteer scribe at a private clinic. I started in Dec. 2013 for about a month and then stopped. I began volunteering at the same clinic this January and plan to continue until end of summer. I think I will have a total of 100 hours by application time.

On AMCAS, can I put the time range for this activity as Dec. 2013-June 2016 with 100 hrs. or no? Do I have to relay the discontinuity in the activity?

With a gap that large, I would definitely make it obvious that it was discontinuous.
 
That's a significant discontinuity, and it should be noted. It would be dishonest to imply that you had worked there from Dec 2013-June 2016 when the reality is that you only worked there Dec 2013 and Jan 2016-June 2016.

How badly would this affect the clinical experience part of my application? Considering that this was broken-up, will it be a red flag even if I have 100 hrs of it? I do have lots of shadowing and hospice experience too…

I'd be concerned if this was your only clinical experience, because it's obvious you are doing lots of hours in about 6 months just for your application (I've heard you should aim for ~150 hours over 1.5 years), but you say you have lots of hospice experience which could help alleviate this.
 
Either list them separately or don't mention the December 2013 stint. 100 hours is 2 1/2 years would look rather strange. That would be like 3 hours a month. You could indicate you plan to consider this activity throughout your app cycle.
 
I can't really remember, but I think there is an option in AMCAS if you did the same activity with gaps in between (ie it lets you put in multiple start/stop dates for an activity).
