if they want to see how you are doing, do they every request fall grades for senior year before making a decision

I have never heard of this. If you did well this semester find out if they are open to updates and send them an update. If you did not do well, don't update.

It's school-dependent and you should check with the school. Some schools will say that you can send them an update about fall grades. Other med schools say specifically that they don't want grades as sole updates and they'll make their decision based on the grades they have available to them already.
