Do ECs still matter for top MD/PhD programs?

Obviously research is the largest component (length, quality, publications, etc.). Meanwhile top-tier MD programs demand wild, interesting ECs. What is the case with top MD/PhD programs? Do they want stacked research AND great ECs (fml)? Or do they still focus mostly on research and ECs are a passing consideration? Thanks.
