I have a DO that will write a recommendation for me but he said he would rather fill out an evaluation form. Does every school have an evaluation form or just some of them? Also, I read on another thread that the evaluation must be accompanied by a LOR. Is that true for every school? Lastly, I have an MD that I have shadowed much longer. Would it look weird if I sent the LOR from the MD rather than an evaluation from a DO? Thanks
