Do I need a rec letter from something health-related?

I've got 1 volunteer (non-clinical) and three faculty letters. Over the past couple of years, I've had a number of conversations with, and have sought advice from, a doctor who is a family friend. I'm worried that our relationship is too informal, but I also wonder whether my application will suffer because it lacks a letter from something directly health-related.

Any thoughts?
