Do I need to give personal statement to dentist writing me LOR?

I recently asked the periodontist that I've been shadowing to write me a letter of recommendation and he said he'd be glad to. Unfortunately I haven't finished my personal statement and so I was just going to give him my CV. Do I need to give him my personal statement in order for him to write me a letter or is this not usually required? Please let me know thanks!
 
Nope, nothing is really 'required'. Additional information always helps though!
 
I gave my recommenders a sample of my writing that included my motivations for dentistry, motivations for my major, my extracurricular activities, and my background. They all said that it was extremely helpful in helping them write their letters. But I also go to a huge university where professors write letters for people they don't really know all the time.
