Do LORs go directly to AMCAS or individual schools?

I am not applying until next year but I'd like to get letter writers earlier and save them in Interfolio. Do they write the letters generically to AMCAS or do LOR go to individualo schools? Can someone pleasse give me a sample how a letter is written. I mean do they address it "To Whom It May Concern" and then write "I recommend Dr. Josh to any med school blah blah blah" or does it have to be addressed directly to the school. Some writers seem to want to wait until I know where I'm applying. I'm afraid they'll forget me by then.

I also have a concern that in large science classes those professors don't really know me. I'm not going to pretend that I don't understand something just to raise my hand or come to extra help; they don't ask other questions and they are large classes. What do I do to get a professor to know me. Also what if I ask ahead of time for my orgo prof to give me a recommendation if I get an A in the class. I have a high A now but will not know what I ge ton the final until classes are over and he will not be around the rest of the summer. I suppose if her agrees to write it I can put it in Interfolio and just not use it if i don't get an A. I'm not sure what to do, he doesn't even know my name though we have spoken and he referred to me when speaking to someone else in class as "your learned collegue". 😕

I only have one more science course to take and so far no definite letters. One was supposed to write me a letter but she has not responded to my e-mails once classes ended. I had a 98 average going into the final and somehow ended up with a B+, so now I'm not sure if I'd even want her to write a letter. I was shocked and e-mailed her twice asking what I actually got on the final. I got an A in her other class. What should I do?

LOR letters go to individual schools. Addressing them "To whom it may concern" is appropriate. If you think it is okay to get a letter of recommendation from someone who doesn’t even know your name then I don’t know what to tell you. A LOR is more than this student got an A in this class.

It was perhaps an exaggeration to say he doesn't know my name, but it's not like he knows me or anyone well. It's summer school and I've only had class for 12 days and he took off two of them.😱 And two of them were exams. Four more days left and one will be the final exam. I don't have too many choices either. Most of my science classes were taken abroad over a year ago and others were explained above. 🙁
