You may have to clarify what you mean by academic hospitals and community hospitals. If by academic you mean being a university medical center (i.e. Ohio State University Medical Center), then you won't really find any in that regard. Then again, MD schools with
those kind of medical centers aren't necessarily pumping all their students through that, but instead also rely heavily upon "community hospitals". If by academic hospital you mean that it's a teaching hospital with residency programs, then then are a plethora.
The following schools I know give you the option to be at teaching hospitals:

There are more, but these are the ones off the top of my head. There are places like Pikeville that I really have no idea
how many spots they have at teaching hospitals, but I know that they
do have them. When I'm talking about teaching hospitals, I'm refering to places like Genesys (
www.genesys.org) in Michigan, Doctors Hopsital in Columbus, Grandview in Dayton, and all the other hospitals that you'll see affiliated with residency programs on
http://opportunities.osteopathic.org . Also, several places also rotate through "allopathic" teaching hospitals, such as KCOM students rotating through Southern Illinois University's programs in Springfield, IL (St. John's, a 738 bed hospital; Memorial Medical Center, a 524 bed hospital), or Via Christi Regional Medical Center (an 840 bed hospital in Wichita, KS) which is where KU has a lot of their programs and sends their students.
So if that's what you mean by academic hospital, then there are plenty of options. However, I don't know how each school works their system/lottery and how many people have to rotate at other places that aren't "academic" hospitals. At KCOM, you'll rotate through an academic hospital if you want to, as there are plenty. You may not get your first choice, but the odds of getting your first choice are generally better than not getting your first choice. Other places that run their own programs, such as MSU, will have plenty of options of hospitals to rotate through.
