Do they know if you have applied to their specialty in the past? Is there some way they can see that info on their end through eras?

No, although we keep ERAS databases for a few years. If you applied to our program in the last 2 years, and esp if your name looks familiar, I'll check to see. If you applied to other programs in the past, there's no way I'd know.
 
Maybe you can offer more feedback AProgDirector. Here's why I ask...

A med school buddy of mine (no, it's not me) applied to two specialties last year. He even interviewed for both. Come match list time, he decided to go with one over the other. Now he's a third of the way into his internship and he's having serious misgivings about his choice. He's seriously considering re-applying for the other specialty that he originally decided against.

And this brings up his dilemma. If he were to re-apply, he doesn't know if he should be up front about how he already applied to and even interviewed in this specialty before. He's afraid that will cause the programs to be very hesitant about interviewing him (understandably so, in my opinion). On the other hand, just portraying the picture of the resident who is just now changing his career path is a bit more common and easy for PDs to be sympathetic about.

Therefore, is it better to just be up front about it, or is it better to keep those details to himself? Also, I wonder if all PDs remember/keep records on previous applicants. He'd be applying next year if he decides to do so.
