applying this year.... havne't done much as a med student. did bunch of research/activities in undergrad. is it ok to include undergrad stuff on cv?

Sure, as long as you feel its relevant to your current goals (ie, I wouldn't include lifeguarding duties and the like). But definitely include research, publications, medically related stuff even from undergrad.

what about nonclinical community service? is that pushing it?

no its not...list it...
i know people who put HS stuff down...and their SAT scores... (field required it...)
