docs in private practice

i'm an MS3 and have pretty much decided that i'm going to go into anesthesiology, so i'm looking for more info about the field. i've shadowed several docs during surgical cases in the hospital, but i've been wondering -- what do docs in private practice do? are they contracted out to hospitals or do they work in outpatient surgical clinics? also, is a fellowship required or is that only if you want to subspecialize in pain, trauma, etc?
