Yes I would put them down, with the Dr. as the contact.
 
Yes if you need the hours - not your fault. Unless you had some reason to know shadowing would be problematic.
 
Just curious, what rule did they break?

It might be that the facility has a rule that physicians cannot have visitors in the facility and that only authorized personnel can be present in patient care areas. I could see a hospital having a rule like that for liability reasons.

That said, it is unlikely that the adcom knows that a facility has such a rule or that the physician who was being shadowed was breaking that rule.
 
This is why I was not a fan of shadowing in general. I loved seeing how medicine worked, but I generally felt so out of place, intrusive, and useless when I did. Loved the learning experience, but despised the discomfort of feeling like it wasn't my place to be there with zero clinical training under my belt.
