On the document submission form
Does the Document Submission Form also need the attending's signature when he/she writes my LOR? According to my understanding, the attending only signs the LOR and the LOR cover letter. But, in the sealed envelope the attending will put the LOR, LOR cover letter and the document submission form, right?
