Documented observation hours ...

Hi,

"Documented" observation hours are required at some schools. What does it mean by "documented"? It is just a letter from a dentist stating the hours observed, perhaps including his license #? I've asked the dentist I shadowed and he had no idea (it was not required when he was in dental school).

Just a letter from the dentist you shadowed stating the numbers of hours.
You don't need his license number or anything.

Something like:

To whom it may concern,

This letter is to confirm that _____________ has shadowed at ____________ for _____ hours. Procedures observed include ______, ______, _____, _____, _____. If you have any questions or comments, please do not hesitate to contact the clinic.
