Documenting illicit drug use?

I am wondering if anyone knows of any opinion/position/research papers about documenting illicit drug use in your h&p. It is something that I have mixed feelings about and would like to know a bit more about (and would like to incorporate into a presentation...)

I have tried my friends Dr. Ovid and Mr. Google, but have not come across anything legit/specific.

I feel like it opens the patient up to judgment from SO many people reading their chart and making decisions about them. I was watching a tv hospital show and this kid comes into the er passed out after going to a club. The doctor was such a dick to him until he found out that he had a problem not related to drugs. "Were you doing any drugs tonight"" like ten times right to him, each time he said no. WHen he got the tests back he realized it wasnt drugs, and IMMEDIATELy became mr. sunshine. I thought that was so lame, treat everyone the same, i mean not murderers/rapists. But even if he was on drugs it wasnt his intention to get to the point of going to the ER.

But you DO need the relevant info for interactions and stuff. SO what are you going to do not list something they tell you? I would list it to be safe, but not judge.
