Documenting shadowing hours?

Hello guys,

I haven't start shadowing yet but i will next week. Since I'm behind, i'm thinking of shadowing multiple dentist to get my hours. And maybe hopefully one of them be cool and nice enough to let me shadow weekly from now till mid may then I can get a recommendation letter. But regarding the hours, do the dentist have to document it or you just fill out of how many hours you did and give contact info of all the dentists you shadow?

I kept an informal record of my shadowing hours. The only school that I applied to that required exact times, contact information, etc. was OSU: they had a specific form that I had to fill out and have my dentist sign, affix his business card, and mail to them. Most schools are satisfied that you can just put down that you have shadowed.
