documents submission to ECFMG

I'm a IMG preparing the envelope to sumbit to the ECFMG office. I'm not sure of what I should put inside. I put my form 186, a passport-sized photo and a letter signed by an authorized official stating that I met the requirements to receive the degree, which will be issued [date]. What else should I put in the envelope? Do I need to add form 344 and two copies of form 345 even if I'm not actually submitting my final diploma? Am I missing anything else? Thanks.
