Does anyone have a good screening question for mania?

I don't think there's one good screening question - you have to run through the criteria. DIGFAST is the mnemonic I use:

Distractability
Indiscretion
Grandiosity
Flight of ideas
Activities increased
Sleep decreased
Talkative

Another mnemonic I'm aware of is DeTer the HIGH

Distractability
Talkative
Reckless behavior
Hyposomnia
Ideas that race
Grandiosity
Hypersexuality
 
Have you ever had periods of times when you were the opposite of depressed?
Intro question to lead into specific questions... if a person has had an all-out manic episode in the past, they'll know what you're talking about.
 
I start my questioning off with "Have you a need for a decreased amount of sleep, an increased sex drive, and have you felt exceptionally cheerful/happy in last few weeks?"

Then I probe further if I get a positive answer or a wishy washy answer.
 
You know, I posted a joking response, but forgot to post the question line I use: "How are you sleeping?" It may not be great, but that "reduced need for sleep" often suggests whether or not to r/o bipolar. I mostly deal with people who have anxiety problems, which can look a lot like mania sometimes. If they say they're not sleeping, "but I don't seem to need it," that tells me to do a little more prodding. If they say they're not sleeping and can barely function, we can move on...

Not great, but sometimes somewhat suggestive...
