Several friends of mine, including those at Columbia, take one non-clinical day a week on average under the auspices of it being a chance to catch up on non-OR learning.

How common is this? Do most anesthesiology fellowships have a non-clinical day?

Sounds pretty cushy. I'm jealous. I've never heard of it anywhere.
 
This really is not that odd. These "reading days" serve a number of purposes. They can be used for clinical research, job searching, or actual reading. This dedicated non-clinical time is especially useful if you are preparing for the boards or trying to learn a skill like TEE.
By doing a fellowship you are sacrificing a year of your life to gain additional subspeciality training or skills. You are also forgoing the income you could make as an attending. These non-clinical days are deserved.

Fellowships at my institution give you an extra day of work instead.
