hey everyone,

i've been lurking for a while and have always totally admired many of you and structured some of my studying after considering all of the advice on this site.

i'm studying for step 1 and i have about 3 weeks left. i am finished with UW but i only did the questions in tutor mode and not randomized. i have QBank and i'm wondering if at this point i should try to finish it, all sessions timed and randomized or if i should redo UW in the same manner. what do you guys suggest?

I would do kaplan and maybe a little bit of USMLEworld to make sure you remember your stuff. Eventhough usmleworld is godly around these parts, I cant deny that kaplan helped me a lot. since you have the luxury of extra time, take advantage of it and do more questions.

do it again...make sure u know that cold! 40% of the test is that difficulty level. 40% of the test is the level of the 150 releases Q's
 
I'd say you need to do SOMETHING to work out timing on Step 1. I am ALWAYS done with tests very early and I found myself using all but 2 or 3 minutes of my time for each section of Step 1. Whether you re-do UWorld or do QBank, make sure you do it timed for the next few weeks.

I'm inclined to tell you to move onto another questions source, but I'm torn because I think UWorld is sooooooooo much better than QBank. I'd probably say go for UWorld. Good luck!

I'd say use Qbank to fill in your weak spots. Really I felt that Qbanks behavioral was great and really helped me iron out a lot of my weaknesses. Otherwise, do the questions you got incorrect in world over again timed. It'll make sure you remembered material and learned your first time through. But that being said, at a point it's a waste - if you're getting more than 90 percent right dont do them anymore.

Actually, in hindsight I think Qbank realllllly helped me iron out concept differences for subjects in which there just arent enough questions in World. Like Skin - after doing the 20 Qbank questions on PV versus BP I know them cold - and yes there was 1 question on my exam.

I have a very low opinion of Qbank. If the test is anything like that, I think I could take it drunk and pass. Most of the Qbank questions I fly through in about 10-15 seconds after scanning the question stem and get in the 75% range...Furthermore, there are some errors in the Qbank. Just today, had question about the most common osteomyelitis in a diabetic's foot and it misidentified the correct bug as S. Aureus (it's Pseudomonas in Diabetics)

...UWorld is quite another matter.

It's S. aureus.

Clin Infect Dis. 1997 Dec;25(6):1318-26.

Osteomyelitis of the foot, a common and serious problem in diabetic patients, results from diabetes complications, especially peripheral neuropathy. Infection generally develops by spread of contiguous soft-tissue infection to underlying bone. The major diagnostic difficulty in diabetic patients is distinguishing bone infection from noninfectious neuropathic bony lesions. Certain clinical signs suggest osteomyelitis, but imaging tests are usually needed. The 111In-labeled leukocyte scan and magnetic resonance imaging are the most diagnostically useful. Staphylococcus aureus is the most common etiologic agent, followed by other aerobic gram-positive cocci. Aerobic gram-negative bacilli and anaerobes are occasionally isolated, often in mixed infections. Antimicrobial therapy is best directed by cultures of the infected bone, obtained percutaneously or at surgery. Antibiotic therapy should usually be given parenterally, at least initially, and continued for at least 6 weeks. Surgical debridement or resection of the infected bone, when feasible, improves the outcome. With appropriate therapy most cases of osteomyelitis can be successfully managed.

There is a lot of pseudomonal colonization of diabetic foot wounds, but little evidence that they are the top cause of acute osteomyelitis.

Nevertheless there are lots of organisms that can cause osteo in diabetic feet and I don't think the evidence is so strong that Step I would make you identify an organism on clinical presentation alone without additional data.
 
I want to second liking QBank's behavioral section. Their question stems are very long and they have a billion options, but they really give you solutions from every angle that really help you understand what they're looking for in an answer on the test.

Tend to agree with this as well.

One downside I found with world is that the questions aren't as long as the real thing, making it hard to get the timing right. You will feel like you are rushing on the real thing.
 
Right you are...S. Aureus overall, P. aeurginosa when feet are punctured through rubber footwear, Salmonella Paratyphi in sickle cell...

....UWorld dinged me on a question with regards to that.
