DOT forms

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
I have no relevant expertise to opine on liability but I have filled out a DOT form for a CDL holding patient who was on a few meds who I did not think was having or likely to have any alertness issues from the regimen. Pretty much all of our stuff is potentially sedating and if other people have some sort of rigorous thing they do for this I'd be curious to find out.

Aren't there specialized clinicians who do this?

I agree pretty much any medication could have sedation and also feel like I am not trained to fill out this DOT clearance then why should I take that liability. Thank you for your input.

I believe there are primary care physicians who also does DOT clearance and that's what I advised to the patient. I would be happy to provide mental health treatment records but I would think they can fill out based on records.
