at einstein, which of these is the better regarded program...ie the more competitve one...its montefiore right?

People go to each for very specific reasons, since they are very different programs. But I suppose if I had to pick which is the more highly regarded, it would be Monte. Jacobi is known as a "unique" place, in that you have to really like that type of program in order to want to train there. It is known to be very "gritty", being in an old hospital with a large patient roster. Though Monte is also huge and has lots of work, Jacobi is known for that "thrown in the fire" feel. That said, Monte is known for working its residents very hard. The schedule and the work load is known to be the heaviest of the Einstein programs.

i heard that Jacobi has a new hospital...i wonder how it is...probably pretty nice?

In regard to the Monte vs Jacobi question, Montefiore has recently been regarded as the stronger program. In fact, many people who I have spoken with say that Monte is the 3rd best program in NY (behind Columbia and Cornell) but doesn't have the rep because of its location in the Bronx as opposed to Manhattan where all of the other major NY programs are.

I also heard, though, that Jacobi is building a new mega medical center and, along with the top rate training that occurs there, will probably put Jacobi "back on the map" with the big boys.

I am applying for IM and want to stay in NY so I've been doing a lot of investigating. Hope this helps.

hey rajvosa,

first off that was a good breakdown....

secondly, is either one better for hem/onc? i know its not as competitive as cards, gi but i was still wondering....

third, jacobi, how much of house staff is img, are we talking most of it, or just a few?

thanks..

Monte also has a very unique call system. I don't recall the specifics, but it is something to the effect of going home for a few hours on call days, and then coming back in the evening to do overnight (after some sleep). Kinda odd, but supposedly it works. rajvosa may have more info on this than I do; The other thing is the Jacobi ER...it is kinda nutty. They have a crazy system where CXR's are done by a list, and the name of the patient is called out...so in the middle of the craziness, if you don't hear your patient's name, you miss it. Then you go back to the bottom of the list. We have combined ER/IM residents that rotate through Jacobi and they say that medicine there is...well, let's just say it's a "challenge". But some people want Jacobi because, as rajvosa says, you pretty much do what you want since there are no privates. But the down side to the whole municipal city hospital thing is that nursing is crappy (and they aren't always that nice to the residents), there are sometimes 6 patients to a room, and ancillary is not great. For instance, some have told me that after a certain hour at night, the intern is essentially the IV person, the blood draw person, the transporter, etc....you get the picture.

New hospital will have 1 or 2 patients per room.

does that mean there is no night float system at montefiore?

I am still not clear what this Monte call system is...

If you are on call during the day you are on call till 9pm. Then you give your board to the night flow. You will pick up the board at 7am.

Now, you may be on call overnight for the whole month (q4). THen you admit from 9pm-7am q4. However, on your call day you go home at 12 noon and come back at 9pm and then admit till 7am. This way they avoid paying penalties for keeping people more than 24hours in one shift.

Yeah, something like that...I remember when I interviewed there, they tried to explain it, and everyone was scratching their heads. They claim it really works well, but the residents seem exhausted. The main things I remember from my interview day are 1) really huge program, 2) unique call system, 3) really friggin' hot in the hospital! (I was sweating my ass off...and it was winter).

Monte is the only hospital in NYC with positive balance. Every othet hospital loses money.

Elmhurst is an HHC hospital, and is also part of the "Queens Health Network" (with the Queens Hospital Center, of late immortalized in "Spider Man" (whereas Elmhurst was the hospital 'Noah' got taken to in "US Marshals" after he got shot)). Jacobi is also HHC. That is why there are McD's in some hospitals (Jacobi, Elmhurst, I think Bellevue), and BK's in others (Coney Island is one).

The point of my post is that Elmhurst would make a profit, if Queens wasn't attached - Elmhurst subsidizes QHC, so there's a net loss. Also, the computer system at all the HHC's is a variation on HDS, and all HHC's have PACS (radiology), so it's up to individual hospitals to fine-tune it; Jacobi does it very well. Elmhurst is hooked up with Mt. Sinai, but the education isn't up to the MSH level; however, just like Jacobi, there are no privates, so you have free rein.

Sorry for the diversion.

There are community hospitals in Brooklyn that turn a profit.

according to a friend of mine who is a resident at cornell, they're in the black these days. i haven't spoken to the board of trustees to confirm yet so take it for what it's worth.
