Hello everyone, I am looking for assistance on this EKG, thanks! 🙂

Preface: 56 yo female , h/o CAD s/p DES to the proximal LAD and RCA with CTO 2008, EF 15% (no AICD) presented to HH ED with cardiac arrest, she called EMS was found lethargic and gasping resuscitation started and continued in ED for 25 minutes (PEA initially then VT and was shocked twice) succeeded in restoring ROSC , after intubation, therapeutic hypothermia started. She was transferred to ICU were she was found to have multiple lab abnormalities including a K of 6.8.


(90bpm, normal rhythm, no p waves, left axis deviation (RVH), RBBB, peaked T wave from hyperkalemia, some ST depression)

What is the commonest situation in which this rhythm is seen?
