Had a pt: 3 weeks gradual onset low grade unitesticular discomfort. Relieved with scrotal elevation. Unprotected sex recently with reported possible Chlamydia exposure in the past. Afebrile no dysuria or discharge. Normal testicular lie. Tender epidydimis.

Would you get U/S to r/o torsion in this case or just Tx for epidydimitis? What follow up is needed?

...that and the usefulness of eponymous tests like ?Phren's (lifting nuts and making critical decisions on such low-sensitivity/specificity nonsense is nuts👎thumbdown) are, IMnot soHO, useless in a modern ED...equivalent to carefully assessing for Beck's triad --> just slap a damn ultrasound on! and stop glorify elderly docs who quote esoteric, irrelevant literature from the dark ages!

These are testicles we're talking about.

Don't know about you, but I don't want to be up late at night worrying about some guys left nut. Just do the US
 
Can we all just admit this is purely playing defense. Treat what we all know it is, no imaging needed. Maybe I just need to be stung then will be playing more defense.
