Is anyone familiar with any literature that addresses the possibility of surgical instrumentation and/or local anesthetic being the sole cause of epidural hematoma (during epidrual blockade), i.e. can the local anesthetic induce a pathological state which predisposes to hematoma formation?

the only thing that has been shown to be linked to epidural hematoma formation is active anti-coagulation or some bleeding diasthesis
