I remember on the form to obtain training permit/limited license (PGY1) it did ask if id ever had DUI specifically

& yeah it had another question about substance abuse

Not rly sure about ERAS but you'll def have to disclose it on state licensing as well as DEA credentialing i believe. A DUI is a big deal in most states
