ERAS Fellowship: Where do I get document saying I didn't read letter of rec?

Planning to apply to cards. Just got my eras token. Is there a section that lets u print a document saying that u've agreed not to read what the recommender writes? I remember signing something like that for residency applications. thanks.

The waiver you need to sign is on the LOR cover sheet that you need to download from ERAS. One cover sheet for each LOR.
