Estranged parent/how to fill out on application?

I have been estranged from my mother for two years. It's a long story, but basically she is a horrible person and my father and I both decided to finally cut ties with her. Should I list her status as "unknown" on the application? I don't want to input her address, phone number (I don't even have it anymore) or any other details. I do not want her name any way involved with my dental school application. Would that be a fair way to categorize her?

I too am in the similar situation. Regardless, they are your parents and the closest bond that you have.🙂
I wish humankind are understanding but most likely they will judge if you don't get along with your parents or unforgiven to them....how will you deal with 70-180 strangers.

They don't contact your parents, just fill it out if you have the info, if not, put unknown.
