evaluation forms

Are these evaluation forms that some of these school have required? I know they "say" that they are optional, but should I go ahead and send it in as well? If I send in one of these evaluations, do I need to also send a letter from the professor?? Please let me know a.s.a.p

As I recall from last year, MSU-COM sends forms that your evaluator must fill out, and it is required. I'm not sure if they would take an additional letter from the evaluator.

I just spoke to the ppl at KCOm, they say that to fill out their evaluation form is not required, but even if it is filled out, a personal letter must accompany it. So, I got one prof. sending a letter to 4 schools, I don't want to burden him with any more unneccessary junk if I don't have to.....so what should I do??
