should i fill this in even though my committee is sending all my letters on my behalf?

Also, does this include my Professor's evaluations? Or just volunteer experiences?

EDIT:

i tried creating evaluators but it wont let me create more than 1 while having a committee letter. SHould i just create one evaluator and have my committee send out all my letters?
