So I am applying to a medical school without using the AMCAS since the program I am applying to has its own application site. So how would I deal with these issues when I can only enter a start and end date and hours/week.

1: I took a break for a few months in the middle of my start and end date, should I just mention this in the description?

2: Some of my activities took place once or twice a month for almost the whole day, rather than weekly. Should I average the number of hours per month and divide that by the number of weeks in a month to get the hours/week, or should I mention in my description that the activity happened once/twice a month along with the amount of time I spent for those one/two days every month?

This is how the AMCAS application was some years ago. You were expected to divide your total hours for the entire timespan by the total weeks and give the average in hours/week. In your case, this does not represent a true picture of how you spent your time. It will also significantly dilute your hours per week when you took three months off in the middle of the timespan. If they give you sufficient space in the narrative area to explain, I'd try to give as true a picture as possible of how your time was actually spent (as you've explained it here), even though you also fill in the averaged hours/week space, as required.
