In my hospital, we are using the add-a-vial for many piggybacks. We stamp it with expiration one month after it has been prepared. Lately, there has been a lot of piggybacks expiring mainly because techs are not rotating stocks when refilling the pyxis... Our supervisor is waiting on word from manufacturers on how long the piggybacks should be good for. How long do you think is the proper expiration for IVPB that have been prepared using the add-a-vial system? Is one month too short, about right or too long?
 
Extending the expiration of the Add-a-vial or Vial Mate isn't the solution for you guys... because if your workflow doesn't allow for rotation of stock or ineffective batching system where you overprepare stock, then the stock will expire...2 months later instead of 1 month.

Baxter will recommend 30 days.... this is not a stability issue rather a sterility. Also, remember, The Joint Commission will mandate that if a product is available commercially prepared (ex Frozen Piggy Bag Zosyn 3.375 grams), then you must opt for it instead of making an Add-a-Vial with 100ml NS and a vial of Zosyn 3.375gram. For that... TJC can go F*CK Off.... Why the heck do we need USP 797 if we have to buy everything.

i agree with epic. it's not the add-a-vial that's not working for you...its definately your workflow. look at the usage for each pyxis station...if you arent using the stock in a timely manner, it's more than a matter of not rotation stock.
if you are filling pyxises at least twice daily, there is no reason to load it up with tons of stuff that will take up too much space and expire. also, you can enable "outdate tracking" on each drug...you can then report on what will expire when.

we use 30 days for 100ml and 15 days for 50ml per baxter's recommendations.

also, the JC can go scratch itself.....🙄
