I can imagine it being difficult if the reason for withdrawal is a reason that may repeat itself and subsequently categorize the person as an ongoing program attrition risk. This difficulty may be made worse if performance indicators, such as GPA and MCAT, do not support a conclusion that the student excels when said medical cause is not present. Conversely, if said medical cause will never again manifest or if there is no obvious risk to the student's performance, then there should be minimal difficulty.
I would strongly recommend finding a way to turn the reason for the withdrawal into a further motivating factor for "why you, why medicine?" Whatever it is or was, there's a good chance you can manage this. The people getting multiple A's likely have something on their app that makes them different from everyone else. With 5,000+ people as your average competition, this withdrawal might actually be the eyebrow raise that gets you the A over someone else. Context is everything.
