How can the insurance company partner up with a compounding pharmacy? They have to provide a different dosage each time right? You can't just outright provide the same dosage each time right??

They are providing the drug in combo with leucovorin, so they are not in violation of Daraprim's NDA.

Insurance companies partner with pharmacies all the time, it's no different than a specialty pharmacy contract.
