I am a rising MS4 and just wanted to know if anyone has any input into whether to do a medicine or surgery externship 4th year(if our school doesnt offer one in ophtho)? also, is it necessary to do your externship early in the fall(to get a LOR)? Thanks

My understanding is that externships are away rotations.

Because it's away, you ought to pick a school that has an ophthalmology course to do your away rotation at.

I would consider doing it as early as possible in order to get a LOR.
