Ive looked but nothing ever equals what I can make with an extra call or staying a bit later in the o.r.

reps? Ask a company who the rep is for your hospital for whatever thing you think is wonderful. Then ask the rep if they are looking for speakers. I had the local Ofirmev rep begging me last month to do speaking engagements for her (despite the fact I give out PO tylenol like candy).
