So I heard that on the application you put the start and end month for each activity and the average number of hours per week.
But what if I took a 4 months break from something like research? Would I just average the hours accordingly? Also for activities that happen during school semesters and doesn't include summers?

You have a few options
1. You can put down the most recent time span in the dates field, then describe the other time spans in the description for the activity
2. You can figure out your average hours per week for the whole time period including the 4 month break, like you suggest
3. You could put the real hours per week and total time span but make it clear in the description that you take the summer off

Normal school breaks are assumed but summers off should be noted in some way
