"observed"? Did you just go to different pharmacies and observe what the pharmacist does? If so, I don't know if this is considered volunteer or extracurricular activities.

I would put them all in one. Otherwise it looks like you're just trying to fill up space. And really it's all the same except in different places. But if you want to spread them out it doesn't matter.
