Can a clinical fellow apply to the NIH F32 with plans to do 80% research and 20% clinical? The application instructions make it seem like the answer is no, but I'm still curious. Is there any way to make it work?

Yes. However since those grants are 2 years, you will likely be required to extend your time in fellowship to make sure you get the required clinical time (similar to a T32).
