Step 1: 234, havent taken step 2 yet

failed my FM shelf by one point by my schools standards, it was my second rotation overall. They're saying its udnerstandable sicne I haven't had medicine or peds or surgery yet (only psych...). Any plan of action on this guys? Am I doomed if I want to get into a generic program in NYC? I obviously need to rock out my other shelf exams and Step 2. Thanks in advance.

You're dead on arrival chief. Gigantic red flag.
Your only hope is to match into family medicine...

J/k. You'll be fine if you turn it around and do well from here on out. Everyone has an off day. And it's FM ffs. I'd give ya a pass on it of it was your only issue.

How in the world would people know if you failed a shelf? The only way they could know that is if you failed the rotation as well. Did you fail the rotation? Failing the rotation would be a much bigger red flag than failing a shelf. No one sends out those shelf scores (not that I know of anyway).

No one knows or even cares really. I don't think those scores are noted except at some schools. Usually the shelf is remediated and that's about it.

The shelf exam can be remediated, you won't get more than a "pass" in family medicine but that's not going to hurt you much. Your Dean's letter and transcript will not reflect a previous failed shelf. So you're fine unless you want to bring it up yourself.
