FAP Approval Form - Albert Einstein

I was lucky enough to get approved for FAP from AAMC. Albert Einstein's secondary application says that if you want a fee waiver you need to include a FAP approval form. Does anyone know what this is? I didn't receive any formal letter from AAMC, just an email.

my plan is to print out a copy of the e-mail that the aamc sent me after approving my fap application to all the schools that waive the secondary fee if you have fap. i believe the e-mail says that you can use it for verification purposes. i plan to call aamc just in case.
