Fee Assistance Program Application

Hi, I was wondering if anyone knows how to fill out that fee assistance program application. I entered in my information on the page, but the application would not let me move forward after entering in my income information on one of the pages. It kept saying I was doing something wrong, but I would think I knew my own income etc. Is it even worth it to do the form? My father recently lost his job, but I do not know what income level in dollars falls below the umbrella of the FAP assistance. Any suggestions? Please help!

unfortunately you must report 2004 information. if dad just lost his job, i dunno if they'll foot yoru bill....
